ASSETS ALL RISKS [MATERIAL DAMAGE AND BUSINESS INTERRUPTION] AND POLITICAL RIOT CLARIFICATION OF MAJOR TERMS

Claims arising under this policy normally call for the services of a claims assessor who will adjust the claim in conjunction with the Insurance Broker and the Insurance Company jointly.

Claims Procedure:
We require the following;
1.      Fully completed claim form, signed by the insured (giving full description of loss/damage)
2.      Police report/fire brigade report where applicable
3.      Two quotations for repair or replacement.
4.      Any additional requirements, if necessary.


PROPERTY LOSS & ALL RISKS EXCEPT FOR MOTOR

General Comments:
Under this category come all sorts of property policies except for motor.

Huge Claims arising under these policies normally call for the services of a claims assessor who will adjust the claim in conjunction with the Insurance Broker and the Insurance Company jointly. Straight forward claims may be adjusted by the Broker and settled immediately.

Claims Procedure:
We require the following;
1.      Fully completed claim form, signed by the insured ( giving full description of loss/damage)
2.      Police report where applicable.
3.      Two quotations for repair or replacement of items
4.      Proof of ownership of items
5.      If the item is completely damaged, a report from a reputable repairer must be submitted
6.      Spectacles / Contact lenses. Confirmation from medical aid that a claim has not been lodged against them.
7.      Any additional requirements, if necessary


FIDELITY GUARANTEE

General Comments:
Claims arising under this policy normally call for full involvement of the Police Authority because they are considered as theft being fraud cases.

Claims Procedure:
We require the following;

1.      Fully completed claim form, signed by the insured ( giving full description of loss/fraud)
2.      Police report.
3.      Particulars of action taken against the fraudster.
4.      Claims should be reported within 24 months of discovery.
5.      Any additional requirements, if necessary.

WORKMEN’S COMPENSATION

General Comments:
This policy is compulsory by law under the Workmen’s Compensation Act, 1998 (Cap 46.01 of 1998) (Section 31). Therefore, the administration of both policy underwriting and claims is strictly subject to the provisions of the above Act and is administered by the relevant Ministry of Labour and Home Affairs.

Consequently, all claims adjustments are done by the Ministry in conjunction with the concerned Medical Practitioner’s recommendations to the Ministry. The University’s involvement is of paramount importance.

Claims Procedure:
For an employee injured or killed during and in the course of employment etc.

1.    Initial notification of the incidence has to be made to Labour Office in the particular district in which the injury or death occurred. Please note that this notification has to be done within 17 days by the employer, but not the concerned employee or representative(s).
2.    Notification form from Labour Office should be completed.
3.    The University as employer will subsequently be required to submit further details including completion of claim form.
4.    Death Certificates in case of death claims
5.    Postmortem Report, for death claims, if necessary
6.    Pay slips may be required
7.    Doctor’s Medical Report
8.    Medical Expenses invoice, if applicable
9.    Police Report or letter from the Chief may be required, if necessary
10.    Any additional requirements, if necessary.


GROUP PERSONAL ACCIDENT [STAFF AND STUDENTS]

General Comments:
The claims procedure adopted and documentation required on this section is similar to compulsory Workmen’s Compensation policy although it does not necessarily call for the involvement of the Ministry of Labour and Home Affairs.

Claims Procedure:
We require the following;
1.    Fully completed claim form, signed by you (giving full description of injury/death)
2.    Medical Expenses invoice if applicable.
3.    Death Certificates in case of death claims
4.    Postmortem Report, for death claims, if necessary
5.    Pay slips may be required except for students policy claims in which case the medical doctor’s award is sufficient.
6.    Doctor’s Medical Report
7.    Police Report or letter from the Chief may be required, if necessary
8.    Any additional requirements, if necessary.


MOTOR FLEET – COMPREHENSIVE

Claims Procedure:
For motor claims whether for the staff or corporate the following is the practice:

(a) Motor Damage
We require the following;
1.    A duly completed claim form, signed by the insured, including sketch and description.
2.    Copy of driver’s licence (Driver at the time of the accident)
3.    Police report where applicable has to be submitted.
4.    Full details of other people involved in the accident as well as witnesses.
5.    Copy of Blue Book
6.    2x Repair Quotes
7.    Towing/Charges for protection of disabled vehicle invoices, if applicable.
8.    Medical Expenses invoices up to P1000.00, if applicable.

IN THE EVENT OF TOTAL LOSS, WE WILL CONFIRM THIS TO YOU AND ADVISE OF THE DOCUMENTATION REQURED, NORMALLY :-

1.    Original signed and stamped Motor Vehicle Registration Book
2.    Set of keys
3.    Certified copy of incorporation or Act of Parliament for bodies established by Parliament or Identity Card [Omang] or Passport for vehicles registered in the name of individuals.
4.    Service Book and / or Manual
5.    Any accessories or equipment in your possession  forming part of the vehicle
IT HAS BEEN OBSERVED THAT FAILURE TO PROPERLY COMPLETE CLAIM FORM (S) AND OR TO SUBMIT SUPPORTIVE DOCUMENTS (copies of driver’s licence, quotations etc.)  DO CAUSE DELAY IN THE ENDEAVOUR TO SETTLE CLAIMS QUICKLY. We therefore urge you to procure copies of your BLUE BOOKS, DRIVER’S LICENCE of those authorized to use the vehicle by the motor policy.

The following should be noted;-
i. Your driver/s must not admit liability to Third Parties
ii. No offer of payments must be made to Third Parties
iii. All your drivers must be fully licensed and their licences must be regularly
checked to ensure their validity.

(b) Motor Vehicle Theft
We require the following;

1.    A duly completed claim form, signed by the insured, giving full description.
2.    police report
3.    proof of ownership
4.    2x Repair Quotes confirming that the vehicle is not repairable
5.    Original signed and stamped Motor Vehicle Registration Book
6.    Set of keys
7.    Certified copy of incorporation or Act of Parliament for bodies established by Parliament or Identity Card [Omang] or Passport for vehicles registered in the name of individuals.
8.    Service Book and / or Manual
9.    Any accessories or equipment in your possession  forming part of the vehicle

Note that there is a six (6) weeks waiting period for motor theft claims. This allows the Insurance Motor Theft Investigators time to trace the vehicle and meet the authorities both in and outside Botswana for possible recoveries.

(c) Motor Claims – Windscreen or Glass
We require the following;
1.    A duly completed claim form, signed by University’s authorized official, giving full description.
2.    Two quotations for replacement
3.    Police Report where applicable i.e. in case where vandalism or any criminal activity is suspected and there are possible suspects.

We realize that windscreen claims are of such a nature that immediate attention is necessary. Upon receipt of all relevant documents, (if all is in order) we will authorize immediately and issue the University with windscreen authorization/order.

For claims intimated involving injury/death to third parties, this may be made by the third party against the MOTOR VEHICLE ACCIDENT FUND (MVA).

(d) Motor Third Party Claims
For claims made against the insured from third parties, e.g. under third party section of motor policy, a public liability policy, etc:-

1.    Notify Dynamic Insurance Brokers immediately University becomes aware of any impending prosecution or inquest in respect of any event which may give rise to a claim.
2.    You should refer such communication from third party to Dynamic Insurance Brokers without admitting liability or acknowledgement of such correspondence.


GROUP LIFE ASSURANCE

General Comments:
The claims procedure adopted and documentation required on this section is similar to Group Personal Accident.
